Much as I hate to drag a thread back on course...
Went to Cafe Caldesi in Marylebone at lunchtime and had a very pleasant double espresso. I was sat in a position that enabled me to watch the preparation and passage to my side. Friendly staff even though they were fairly busy; drank up sharpish then mooched on to work very satisfied and abuzz with the caffiene. I imagine they may well take my coffee cash again very soon

Might be worth suggesting Sacred Cafe just of Carnaby Street. They are trying pretty hard though the espresso wasn't quite on the money, didn't see any milk drinks I am afraid.
